This registration session will allow you to complete contact information, sign up for participation, and submit payment.
Please read through each page carefully to ensure that all information is provided accurately.
You will need your athlete's insurance information and a head shot photo at the time of registration.
To complete registration through our secure site, please have your Visa or MasterCard information available.
Completed paperwork must be received prior to June 15th for review and certification. Detailed information regarding required forms will be sent to you directly within the next week.
Additional items to have handy to speed up the paperwork process:
-Original Birth Certificate (with a raised town seal) AND a black and white photocopy of it. *This is not required if your athlete has previously played in Fall.
-Updated physical. *Must be dated from 2024 or the AYF form will require a doctor signature.
-Copy of the End of year report card for the current year. *Must have all terms and final grade completed to be approved.

Practices Kickoff July 29th, 2024 at 5:30PM, Windle Field, Millbury for all teams!
Each family is required to pre-pay a minimum of 5 raffle tickets at $10 each for the season.
Proceeds less prizes help to offset the costs of necessary equipment and league dues in an effort to keep registration price point low for all Raiders athletes.
You may sell the tickets to family or friends to recoup the cost of the tickets, or you may keep them for yourself. We will pull a $50 winner for every day we practice during our August grind, 20 winners in total!
All raffle entry names must be submitted to the snack shack by August 16th, if you do not turn in names by this date all raffle tickets purchased will be entered into the drawing in your Athletes name.
Winners names will be drawn by our athletes at one of our home games on Facebook live!
In the event that we do not have a full team at your Tackle athlete's age level, you will be given local opportunities locally to ensure you athlete is able to participate.
We work closely with the Presidents at all associations in Central Mass and abide by all waiver procedures which have been set in place.
Please note: You must first sign up within your hometown's AYF program before any transfer and waiver can be approved by Central Mass.
